Compare 1-lead and 3-lead ECG monitoring: electrical views, electrodes, signal context, limitations, and when a 12-lead ECG is still needed.
A 1-lead ECG records one electrical view of the heart. A 3-lead ECG records three views at the same time. The additional views can provide more information about waveform shape and help distinguish a true rhythm change from noise, but they do not make every 3-lead recording automatically more accurate.
Short answer: a clean 1-lead recording can be useful for longer-term heart-rate and rhythm monitoring. A 3-lead system provides more spatial context when morphology matters. Neither is a substitute for a standard 12-lead ECG when a clinical question requires one.

The number of leads is only one part of the system. Electrode placement, signal quality, sampling, recording duration, analysis software, and clinical interpretation can matter just as much.
What does a lead mean in ECG?
An electrode touches the body. A lead is the electrical view calculated from the voltage difference between electrodes. That is why the number of electrodes and the number of leads are not the same thing.
In traditional ambulatory ECG configurations, a single lead is commonly obtained with two electrodes, while a 3-lead system may use five to seven. Other designs exist, including compact patches and reconstructed lead systems. Check the exact manufacturer's configuration instead of inferring the number of leads from the number of visible contacts.
A standard resting 12-lead ECG is another category. It normally uses ten electrodes to produce twelve views from different directions. A 3-lead monitor is not a smaller equivalent of that examination.
What can a 1-lead ECG show?
A 1-lead ECG can record the timing and shape of repeated cardiac cycles from one direction. With a clean signal, it can support heart-rate calculation, RR intervals, rhythm review, and correlation between a recorded episode and a symptom or activity marker.
Single-lead recordings can perform very well in atrial fibrillation and other rhythm-monitoring workflows. The useful question is not whether one lead "works", but which device, algorithm, recording quality, population, and reference method produced the result.
The limitation is the single viewpoint. Some waves may be small or difficult to see, and the recording lacks the spatial information needed for many morphology, conduction, ST-segment, and localization questions.
What does a 3-lead ECG add?
Three simultaneous views let a reviewer compare how the same beat appears from different directions. This can make P waves or QRS morphology clearer in one channel than another and can help distinguish a physiological event from an artifact affecting only part of the setup.
That additional context can be useful in Holter monitoring and telemetry, but the benefit depends on correct electrode placement and good contact. More channels can also mean more electrodes, more setup work, and more opportunities for a loose contact. A poor 3-lead recording is not better than a clean 1-lead recording simply because it contains more channels.
Is a 3-lead ECG more accurate than a 1-lead ECG?
There is no useful universal percentage. Accuracy must be defined for a specific task: detecting beats, calculating heart rate, classifying a rhythm, measuring an interval, or answering a clinical question.
For beat timing and longer rhythm observation, a validated 1-lead device may be sufficient. When waveform morphology or the direction of electrical activity matters, multiple leads provide information that one lead cannot. Expert guidance recommends 12-lead ambulatory ECG, rather than a reduced-lead recording, for questions involving detailed QRS morphology, ST-segment patterns, Brugada syndrome, ischemia, or QT dynamics.
The useful comparison is therefore not "less accurate versus more accurate". It is "one view versus more views for the question being asked".

Published results make the trade-off much clearer than a generic accuracy claim:

These numbers describe the tested devices and protocols, not every single-lead ECG. They also show why "3-lead is more accurate" is the wrong shortcut: lead count, recording time, signal quality, algorithm, and the clinical target all change the result.
Which ECG setup should you choose?
For personal or research measurements, consider:
- whether you need the raw ECG waveform or only heart rate;
- whether the device exposes RR intervals and signal-quality context;
- how long the recording must last;
- whether the electrodes remain stable during the intended activity;
- how artifacts and missing data can be reviewed or exported.
For a medical investigation, the recording method should match the suspected problem and how often it occurs. Short, frequent events may call for a different workflow than rare episodes that require weeks of monitoring.
